Overview

What does vacuum therapy address?

Erectile dysfunction affects a large share of adult men, yet it is a health topic that often goes undiscussed. In most cases the cause is vascular: not enough blood collects in the penis, or the blood that does collect cannot be held there. Vacuum erection therapy targets exactly these two points — it draws blood into the erectile tissue using negative pressure in the cylinder, and holds it there with a constriction ring slid to the base of the penis.

The method has been used in urology for more than thirty years. The device introduces no substance into the body; its effect stays local and causes no systemic response. This mechanical mode of action is the main reason vacuum therapy remains an option when medication is unsuitable or does not deliver the expected result.

Active3® applies the same principle with a rechargeable electric pump head: the vacuum is built with one button and released within seconds with the second. The illuminated transparent cylinder lets you follow the process by eye, and rings in five sizes let you find the right tension by trial. After the first few attempts, the whole process takes only a few minutes.

The first step is a medical examination

Erectile problems can be the first sign of cardiovascular disease, diabetes or a hormonal disorder. The device relieves the symptom; it does not change the cause. Whichever method you choose, see a urologist first.

How it works

An erection is a matter of blood volume; vacuum creates that volume directly.

By lowering the pressure inside the cylinder, the device draws blood into the erectile tissue; this mechanical principle works even when nerve signalling or the vascular response is weak. The constriction ring, slid onto the base of the penis, maintains the resulting rigidity — these two parts are the actual mechanism that makes an erection possible.

How does blood flow into the penis?
  • Works purely mechanically

The erection is produced by the blood flow that negative pressure creates in the penis; no substance enters the body. The device acts only where it is applied and has no systemic effect.

  • Within minutes, no planning required

Blood flow begins as soon as the vacuum is built, and an erection forms within a few minutes. There is no need to time it in advance or wait for anything to take effect.

  • Regularly stimulates blood flow to the penis

Regular vacuum use keeps blood flowing into the erectile tissue. Especially after prostate surgery, it helps preserve penile length and girth as part of penile rehabilitation.

  • Built to last for years

A one-time purchase: the device can be used an unlimited number of times, with no recurring cost per use. Rings and other parts are available separately.

  • Electric, for users with limited hand strength

A motorised head instead of a manual penis pump; users with arthritis, rheumatism or reduced grip strength can operate it with a single button.

  • Illuminated cylinder

Lighting inside the cylinder is a detail unique to this product, making it easier to check visually that the ring is positioned correctly.

Application

Six steps, a few minutes

The first attempts are a learning phase; three common mistakes make it harder — not using lubricant, building the vacuum in one go, and choosing the wrong ring size. The sequence below covers all three.

  1. 1

    Preparation

Attach the pump head to the cylinder. Apply a thin layer of lubricant to the cylinder rim that touches the skin and to the penis. If necessary, trim pubic hair so it does not cause vacuum leaks.

  2. 2

    Load the ring

Use the ring loader to fit a ring of the right size onto the end of the cylinder. Starting with the largest ring is recommended for your first attempt.

  3. 3

    Build the vacuum gradually

Seat the cylinder firmly against the groin and press the power button. Negative pressure draws blood into the corpus cavernosum. If you feel any discomfort, do not wait: press the release button.

  4. 4

    Slide the ring

Once sufficient rigidity is reached, slide the ring to the base of the penis. By slowing the return of blood, the ring maintains the erection after the cylinder is removed.

  5. 5

    Remove the cylinder

Let air in with the release button and remove the cylinder. The erection is maintained by the ring; intercourse is possible at this stage.

  6. 6

Remove the ring — 30 minutes maximum

Under no circumstances should the ring be worn for longer than 30 minutes. Once removed, blood circulation returns to normal and the penis returns to its usual state.

The ring stays on for no more than 30 minutes — this limit is not negotiable.

Who it’s for

Who is the device suitable for — and who is it not?

Vacuum therapy is open to a wide range of users because its effect stays local. As with any method, however, there are situations in which it should not be used. The two lists below summarise the information in the instructions for use and do not replace a medical examination of your own situation by a doctor.

Suitable situations

  • Erectile problems caused by diabetes
  • Erectile dysfunction associated with high blood pressure and hardening of the arteries
  • Men taking blood thinners (bearing in mind the risk of bruising)
  • Recovery after radical prostatectomy or prostate radiotherapy
  • Certain neurological conditions such as Parkinson’s disease and multiple sclerosis
  • Men who want a mechanical way to make erections easier
  • Adult men of all ages

Situations where it must not be used or a doctor should be consulted

  • Conditions involving prolonged erections (priapism)
  • Certain blood and clotting disorders
  • Situations in which a doctor has advised against sexual activity
  • Do not use if any part is damaged
  • Do not use under the influence of alcohol or drugs, or in the shower or bath
  • Do not use while charging; never immerse the pump head in water

Erectile dysfunction can be the first sign of cardiovascular disease, diabetes or a hormonal disorder. The device addresses the symptom, not the cause — the first step is always a urological examination.

All 30+ questions

What is a penile vacuum device and how does it work?

A penile vacuum device is a mechanical erection aid made up of a clear cylinder, a pump that draws the air out of that cylinder, and a constriction ring placed at the base of the penis. When the pressure inside the cylinder is lowered, the resulting negative pressure relative to the surrounding tissue draws blood into the spongy erectile tissue of the penis (the corpus cavernosum) and the penis becomes firm. The ring is then slid onto the base of the penis to stop the blood from flowing back too quickly, so the erection is maintained after the cylinder is removed. Because the process is entirely physical, no active substance enters the body.

Is vacuum erection therapy a scientifically recognised method?

Yes. Vacuum erection devices (VEDs) have been used to treat erectile dysfunction for more than 30 years and are listed among the non-drug treatment options in international urology guidelines. They are also widely recommended in penile rehabilitation protocols after radical prostatectomy.

What exactly is included with Active3?

A rechargeable electric pump head, an illuminated clear cylinder (200 mm long, 53 mm inner diameter), ring loaders no. 1 and no. 2, a cone, 5 constriction rings with inner diameters of 9, 12, 15, 18 and 21 mm, 50 ml of Animé lubricant, a charging cable and USB adapter, the instructions for use and a storage case. It is a Class IIa medical device manufactured in Germany.

What is the difference between the electric (Active3) and the manual system?

Both work on the same physical principle. The electric head builds the vacuum with a motor; because it operates at the touch of a button, it is noticeably easier for users with reduced hand strength, arthritis or grip problems. With the manual system the vacuum is created with a hand pump, and the user controls the rise in pressure entirely at their own pace. Active3 additionally has an illuminated cylinder. The pump heads and cylinders of the AES and MES series are compatible with each other.

In which situations does the device make an erection easier?

A vacuum device draws blood into the erectile tissue using an externally applied pressure difference, so it is less dependent on the body’s own vascular and nerve response. It works mechanically even with hardening of the arteries, diabetes-related nerve damage or after prostate surgery, and the constriction ring maintains the resulting erection. Even so, a urological assessment is recommended to establish the cause of the erection problem.

How long can the ring stay on?

No more than 30 minutes. This limit is not negotiable: because the ring restricts blood flow, leaving it on longer can impair the blood supply to the tissue. Remove the ring as soon as intercourse is over; circulation returns to normal on its own.

Which ring size should I choose?

Start with the largest ring on your first try. If the erection cannot be maintained once the ring is on, move down one size; if there is pinching, pain or noticeable discomfort, move up one size. The right ring is the one that maintains firmness without causing pain, and it varies from person to person. That trial and error is exactly why the set includes 5 different sizes.

Is lubricant necessary?

In practice, yes. Lubricant helps the cylinder seal fully against the skin so the vacuum does not leak, and it also makes it easier for the ring to slide onto the base of the penis. The set includes 50 ml of Animé lubricant for this purpose.

How often can I use the device?

As often as you need, provided you follow the instructions for use. Some users also apply it regularly without the aim of intercourse, simply to support blood flow to the tissue; in rehabilitation after prostate surgery, doctors often recommend a programme of this kind.

The vacuum won’t build and no erection forms. What should I do?

Check the following in order: make sure the cylinder sits fully and airtight against the groin, apply lubricant to its rim and trim the hair if necessary; build the vacuum gradually rather than all at once; and review your ring size. To find out whether the device itself is working, you can place the cylinder on your thigh or abdomen and test the vacuum. If pump performance has dropped, check the charge level and make sure the air outlets are not blocked.

Firmness decreases after I put the ring on.

This is usually a sign that the ring is one size too large; try the next smaller size. As described in the instructions for use, it is also possible to reapply the vacuum with the ring in place by using the ring’s tabs.

It is difficult to remove the penis from the cylinder.

Release all the air using the release button. If the difficulty persists, use a larger ring loader or try the cylinder without a ring loader. Never use force at any stage.
